Regional Team Officials Forms
All of the forms you will need
to complete your duties as a regional team official are in this section
of the website. You can print the form directly from the screen,
complete the details by hand and then fax the form to Metropolitan East
School Sport Office (fax 3245 0255); or you can complete the form
on the screen, save the completed form and then email to the Sports
Office as an attached file to:

Forms that require data entry are listed in blue cells. These are required forms, ie, they MUST be completed. Some of the forms require the entry of data only; these can be wholly completed on screen. Other forms require the entry of data as well as handwritten entries such as your principal's signature. These can be completed on screen, then saved and printed out by you for signing prior to forwarding to the Sports Office or wherever. Forms in a third group simply require handwritten entries or no entry by you at all. This group includes statements for completion by the families of students. |

FINANCIAL PROCEDURES AND
UNIFORM ORDERING
All forms must be FULLY completed and returned
to the Regional Sports Office.
No other forms should be
used.
